What is auto-rephrasing in practice questions?

The failure mode rephrasing exists to fix
Spaced repetition is built on the assumption that each revisit is a retrieval. The ratings you give back (Again, Hard, Good, Easy) are interpreted as a measurement of how well your memory pulled the fact, and the algorithm schedules accordingly. That assumption only holds if every revisit is genuinely a retrieval.
On a static card, by the third or fourth revisit your brain has built a shortcut. The first three or four words of the stem are enough to surface the answer because the answer is now associated with the sentence, not the underlying biology. You rate yourself Easy. The algorithm pushes the next revisit out by weeks. On exam day the wording is different, the shortcut is gone, and the answer is gone with it.
Auto-rephrasing breaks the shortcut. The first six words of the stem are different on every surfacing, so the only stable handle your memory can grab is the concept. The Easy rating now means you actually know the biology, and the FSRS schedule is honest.

What rotates and what stays put
The mechanic is intentionally narrow. Surface form rotates. Card identity, scheduling state, and the answer key stay fixed. Anything else and you've lost the spaced-repetition history that makes revisits valuable in the first place.

Where this matters most, and where it doesn't
Auto-rephrasing has the biggest payoff on memorization-heavy material that shows up across many surface forms. Boards exams, USMLE, NCLEX, dental and pharmacy practical questions, anatomy identifications, mechanism-of-action recall. Anywhere the same underlying fact gets dressed up as a declarative question, a clinical vignette, an image, or a negative-form prompt, you want a drill loop that mimics that variability instead of training a single sentence shape.
It is roughly neutral on rote-wording cards (drug names, vocabulary, anatomy labels) where the wording is the point. There the rote phrasing IS the fact, and rotating it would force a rewrite of what you're trying to memorize. Three things to check on any tool that claims to rephrase
- Does the correct option hold across rewordings? If you can rephrase a card and end up with a different correct answer, you've lost the answer key and the card is now testing something else. The rewording is then a content change, not a rephrase.
- Does the FSRS schedule survive the rewording? If a new wording is treated as a new card, scheduling resets and you've lost the spacing history that made the revisit valuable. One fact should have one trajectory.
